Quality Case Management provides services that assist a consumer in gaining access to medical, social and other appropriate services needed for the consumer to remain in his/her own home.
Quality Case Management offers services to individuals that are currently receiving Elderly Waiver Services and are in need of a Case Manager, are considering the Elderly Waiver Program for yourself or a loved one and would like assistance with the application process, or have questions about available services through the Elderly Waiver Program or other available community service options.
The goal of our Case Management program is to empower the consumers to make independent and informed decisions. As Case Managers, we partner with our consumers, families and community agencies in developing Individual Service plans to link each individual with the needed services enabling them to become more connected with the community, independent and safe in their own home. Case Managers play a key role in assisting consumers by providing provider choice options and monitoring services provision.
Initial interviews are conducted with the consumer in the privacy and comfort of their homes or community setting as requested by consumer. Goal directed planning, partnering with consumer’s health care provider and Department of Human Services IM workers as well as communicating and collaborating with local agencies providing the individual with the opportunity and resources to make an informed decision.
